Is it possible to customize risk form (double click on risk ID) within a risk register? I have few items I would like to track.

You cannot change a layout of risk form (Properies tab in Risk Information dialog box). However you add any custom properties to be displayed in the Custom tab of the the same dialog box. To do it:
1. Go to Risk->Settings->Default Properties
2. Enter enty property and define type of this properties (number, string, etc.)
3. Go to Risk Register
4. Enter value for this property in Custom tab of Risk Information dialog box
You may now display and search risks in risk register based on this new property.

I downloaded RiskyProject 5. It has a lot of nice features. Thanks. What I would like to do is to define action items for mitigation plans and be able to report in it. How to do it RiskyProject 5?

Each mitigation plan and mitigation plan assignments have notes. These notes are your action items. When you assign mitigation plan to risk, just fill out notes for each assignment.

I created a custom risk property: "Material requirements". Is it possible to search risk register register to find all risks, where material requirements is greater then from example $5000?

When you in risk register click on Filter button on the left hand side. It will allow you to perform search of the risk register. For example, you select property "Material requirements", define condition (>) and enter value ($3000).
